輔具申請
如果您認為孩子在學期間，各種教育輔助器材(以下簡稱教育輔具）的提供，能夠促進孩子學習及生活適應。教育局提供「教育輔具」的服務。
由專業的評估人員進行輔具需求評估與建議，確認需求後，提供輔具的媒合或採購。這樣可以讓孩子在入學後，就能獲得教育輔具的支持。以下為相關流程與資訊，提供您參考：

教育輔具申請的對象:

(一)就讀本市學校、幼兒園及機構的學生(幼兒)。
(二)經本市特殊教育學生鑑定及就學輔導會(鑑輔會)鑑定為特殊教育生(含確認生、疑似生)或鑑定安置申請中學生(幼兒)。
(三)需使用聽覺輔具之單側聽損學生(幼兒)。
二、教育輔具的內容:

溝通輔具	輔助溝通輔具(溝通器、溝通板、溝通筆、溝通軟體…等)
電腦輔具	電腦輸入輔助設備(軟體、眼控、特殊滑鼠、按鍵…等)
其他輔具	減壓坐墊、吸盤握把…等

*特別說明:眼鏡、助聽器、矯正鞋、背架、餐具、安全帽、推車、重量背心等個人用具，不屬於教育輔具，請家長向本市社會局申請補助經費、自行採購。

申請時間與方式:

孩子目前未就學:

1.將入學幼兒園:
每年1、2月於申請(學前鑑定安置作業)時，由(家長)向承辦單位提出申請。(現場備有表件可索取)
2.將入國小一年級:
每年1、2月由(學區學校)通知(家長)向承辦單位提出申請。

孩子已就學:

隨時由(設籍學校)向承辦單位提出申請。
五、申請資料:
(一)教育輔具服務預約單。
(二)身心障礙證明(手冊)影本(有則檢附）。
(三)申請聽覺輔具者，檢附裸耳與配戴助聽器後之聽力圖。
(四)申請視覺輔具者，檢附視力檢測資料。
六、教育輔具的提供:
(一)開學前輔具會提供給學校，供孩子就學後使用。
(二)若孩子未就學，將不予提供輔具。

Application for Educational Assistive Devices
Dear Parents,
If you believe that providing various educational assistive devices (hereinafter referred to as “assistive devices”) during your child’s school years can promote their learning and daily adaptation, the Education Bureau offers Educational Assistive Device Services. Professional evaluators will assess the needs for assistive devices and make recommendations. Once confirmed, the devices will be matched or procured accordingly. This ensures that your child can receive assistive device support after entering school. Please refer to the following information and procedures:

1. Eligibility for Application
Students who meet the following criteria may apply:

Students (or preschoolers) enrolled in schools, kindergartens, or institutions in Taoyuan City.
Students (including confirmed cases, suspected cases, or those undergoing identification and placement) who have been identified by the city’s Special Education Student Identification and Placement Counseling Committee.
Students (or preschoolers) with unilateral hearing loss who require the use of hearing assistive devices.

2. Types of Assistive Devices Provided

Category	Examples of Devices
Mobility and Positioning Devices	Customized wheelchairs, electric wheelchairs, standing frames, positioning chairs, walkers, gait trainers, transfer devices, etc.
Reading and Writing Devices	Adjustable desks, customized desks (and chairs), reading stands, slant boards, etc.
Hearing Devices	Remote hearing assistive devices (FM systems).
Visual Devices	Magnifiers, CCTV magnifiers, braille machines, screen magnification software, computers for the blind, screen reading software, braille displays, etc.
Communication Devices	Communication aids (communication boards, devices, pens, software, etc.).
Computer Assistive Devices	Computer input assistive equipment (software, eye control, special mouse, keyboards, etc.).
Other Devices	Pressure-relief cushions, suction handles, etc.

Special Note: Personal items such as glasses, hearing aids, orthopedic shoes, back braces, tableware, helmets, strollers, or weighted vests are not considered educational assistive devices. Parents should apply for subsidies through the city’s Department of Social Welfare or purchase them directly.

4. Application Time and Procedure

For children not yet enrolled:
1. Entering kindergarten: Applications can be submitted in January–February each year during the preschool identification and placement process (application forms are available on-site).
2. Entering elementary school Grade 1: Applications can be submitted in January–February each year upon notification by the designated school in the district.
For children already enrolled:
Applications may be submitted at any time by the school to the responsible unit.

5. Required Documents

Educational Assistive Device Service Reservation Form.
Copy of disability identification card/booklet (if available).
For applications of hearing devices: audiograms showing unaided hearing and hearing with hearing aids.
For applications of visual devices: vision test results.

6. Provision of Assistive Devices

Devices will be delivered to the school before the semester begins for student use after enrollment.
Devices will not be provided if the child has not yet entered school.

*The Education Bureau aims to work closely with parents to ensure that every child receives the most suitable educational support and environment after entering school (or preschool).
