在校生鑑定
如果您認為孩子在發展或學習上,可能有特殊教育需求並需要更多支持,教育局提供「在校生鑑定安置」的服務。透過這項服務,由專業的鑑定評估人員進行教育診斷與能力評估,並經由「特殊教育學生鑑定及就學輔導會」討論,確認是否需要特殊教育的協助與安置。這樣可以讓孩子在學校獲得更適切的教育支持。以下為相關流程與資訊,提供您參考:
一、在校生鑑定安置申請對象:
符合以下情況的孩子可以提出申請:
(一) 桃園市公私立國民小學在學學生。
(二) 國小1年級至6年級學生(1年級上學期原則上不受理),且經設籍學校進行校內轉介前介入
輔導後,觀察評估具有疑似身心障礙特徵(須符合《特殊教育學生及幼兒鑑定辦法》之鑑定基
準及《特殊教育法》第3條之規定:本法所稱身心障礙,指因下列生理或心理之障礙,經專
業評估及鑑定具學習特殊需求,須特殊教育及相關服務措施協助之情形
(三) 經本市鑑輔會鑑定安置之特殊教育學生,因學生遇障礙情形改變、優弱勢能力改變、適應不
良或其他特殊需求時且有具體實證,需要申請重新鑑定特教資格及特教相關服務者

知送達之次日起三十日內,以書面向主管機關提起申訴。

In-School Special Education Identification and Placement
If you believe your child may have special education needs and requires additional support in learning or development, the Education Bureau provides the “In-School Special Education Identification and Placement” service. Through this service, professional evaluators will conduct educational diagnosis and ability assessments. The results will then be reviewed by the Special Education Student Identification and Placement Counseling Committee to determine whether special education support and placement are required. - 學校主動發掘具特殊需求學生並完成轉介前介入程序後,由學校提出申請
四、家長需檢附何種資料:
1. 戶口名簿影本。
2. 有效期限身心障礙證明、衛福部認定鑑定醫院或兒童青少年精神科醫師開立之心
理衡鑑報告、聯評報告或診斷證明(非必附,有則附)。
- 申請視聽障組請附聽力值、視力值;申請肢體障礙、腦性麻痺及身體病弱組請檢附
五、不同意安置結果:
1.不同意初步安置結果: 法定代理人或實際照顧者若對初步安置結果有疑義,可於收

In-School Special Education Identification and Placement
If you believe your child may have special education needs and requires additional support in learning or development, the Education Bureau provides the “In-School Special Education Identification and Placement” service. Through this service, professional evaluators will conduct educational diagnosis and ability assessments. The results will then be reviewed by the Special Education Student Identification and Placement Counseling Committee to determine whether special education support and placement are required. This ensures that children can receive more appropriate educational support in school. Please see the following information for details:
Children who meet the following criteria may apply:
- Students currently enrolled in public or private elementary schools in Taoyuan City.
- Students in Grades 1–6 of elementary school (applications are generally not accepted for Grade 1 students during the first semester) who, after completing the school’s pre-referral intervention process, are observed and assessed to have suspected characteristics of disability. The identification must meet the criteria outlined in the Regulations on the Identification of Students and Young Children with Special Education Needs and the definition in Article 3 of the Special Education Act: “Physical or psychological impairments that, after professional assessment and identification, result in special learning needs requiring special education and related support services.”
- Students who have already been identified and placed as special education students by the city’s Committee, but due to changes in disability conditions, strengths or weaknesses, maladjustment, or other special needs (with concrete evidence), require re-identification for special education eligibility and related services.

After the school identifies students with potential special needs and completes the pre-referral intervention process, the school will submit the application.
Parents or legal guardians should prepare the following documents:
- A copy of the household registration.
- (Optional, but recommended if available) Valid disability identification, or reports/diagnoses issued by Ministry of Health and Welfare–designated hospitals or child and adolescent psychiatrists, including psychological assessment reports, joint evaluation reports, or medical diagnoses.
- For specific categories, please include:
- Hearing and vision test results (for students applying under hearing or visual impairment categories).
- Specialist medical diagnosis certificates (for students applying under physical disability, cerebral palsy, or chronic illness categories).
- If you disagree with the preliminary placement result: Parents or guardians may respond within 7 days of receiving the notification and attend a Statement of Opinion Meeting.
- If you disagree with the official placement result: Parents or guardians may submit a written appeal to the competent authority within 30 days starting from the day after receiving the official notice.
